Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing or roll-over hazards:
(a)On or about 05/12/05, at the corner of Deerfoot Parkway, Clay, AL -
Employees
were observed backing TELSTA T-40C trucks into a ditch to make repairs to a
communications line, exceeding the maximum of a 5-degree slope.
Among other methods, one feasible and acceptable method to correct this
hazard is to:
1. Operate the equipment within the parameters of the American National
Standards Institute (ANSI) A92.2-1969, Section 3, Paragraph 3.1.6.
2. Follow the Company training plan, BellSouth Accident Prevention Plan,
MA-BSAP-001BT,Issue 3, dated January, 2003, under Precautions to Observe
before Positioning the
Vehicle, Bullet 4, which states "Aerial Lifts must not be operated on a
slope of more than five
degrees".
