Section 5(a)(1) of Public Law 91-596, the Occupational Safety and Health
Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees, in that employees were exposed to: Being struck by equipment
that are picked up
by hooks without a throat latch and hooks that are used below the lifting
hooks to lift objects
such as steer hubs weighing approximately two hundred pounds and scrap
metal which has not
been tested by a qualified person to determined the rated load, and or
loads suspended from a
electric chain hoist and a two ton crane used to load scrap metal to the
furnace.
(a)Machine Shop: Employees were not protected from the hazard of being
struck by
and /or crushed by equipment and or the suspended load of an electric
chain hoist due
to failure of the hook safety latch not in place to prevent loads from
falling from the
hook.
(b)Furnace Area: Employees were not protected from the hazard of being
struck by
and/or crushed by equipment and /or suspended load of a two ton Shaw Box
crane used
at the pour dock to load scrap metal to the furnace throat latch missing
from the hook.
(c)Shipping and Receiving: Employees using a Gorbel half ton capacity
crane were
not protected from the hazard of being struck by and/or crushed by
equipment and or the
suspended load of an electric chain hoist due to failure of the hook
safety latch not in
place to prevent loads from falling.
Among others, one feasible and useful abatement method to correct this
hazard is to:
Comply with the requirements of ANSI Code, American Society of Mechanical
Engineers B30.10a-1996, Hooks.
Comply with the requirements of ANSI Code B30.20a-1996, Below the Hook
Lifting
Devices, Section 20-1.2 Marking and Construction.
