employer did not furnish to each of his employees  conditions of
employment and a place of employment free
from recognized hazards that were causing or likely to cause death or
serious physical harm to employees, in

and the conveyors
did not have a means of an emergency shutoff switch immediately available
in locations where
the hazard of getting caught by or caught in existed.

provide an emergency stop device that can be immediately operated in the
event of a caught by
