Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injury due to rig collapse: a) Caza Drilling Inc., Rose Ranch, Rig 59, Rifle, CO: On or about 5/23/2005, employees involved in drilling activities were doing so while working from an improperly guyed derrick. The external guyline located in the northeast quadrant of the site was wrapped around a 3" by 3" square tubular leg designed to support the walking surface over a mud tank. b) On or about 5/23/2005, employees involved in drilling activities were doing so while working from an improperly guyed derrick. The guyline located in the northwest quadrant of the site was attached to a portable fuel tank. c) On or about 5/23/2005, employees involved in drilling activities were doing so while working from an improperly guyed derrick. The internal guyline located on the northeast side of the truck carrier was attached to a steel alloy chain in order to gain additional length. Abatement Note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter"). Among other methods, one feasible and acceptable abatement method to correct this hazard would be to follow the Operating Guide for IDECO KM-112-358-AH. Page 1, section I.A.13(d) states to "attach two 1" diameter internal load guylines, on to the bracket on each rear corner of the crown frame. Additionally, paragraph D.2. states to attach the two internal guylines "E" to anchor plates on ramp as shown on diagram on page 6. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the "rigging up" operating guide for IDECO KM-112-358. Page 2 paragraph 14 states the following. Install external guyline anchors spaced as shown in the diagram on page 4. These anchors should be installed to withstand a tension load toward the well at an angle of 45 degrees above the horizontal of 14,000 pounds. WARNING failure to install guyline anchors to withstand tension load as specified, could cause rig turnover, major damage to equipment and or bodily injury. It is recommended the anchors be pull tested prior to attaching guylines.
