Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation and/or crushing hazards: a) Union Drilling Company, Rig 47, Piceance Creek, Rifle, CO: On or about 6/9/2005, the employer did not ensure that wind guys were set up according to the manufacturer's specifications. The anchor installed in the southwest section of the rig was 18 feet west and 105 feet south of the wellbore thereby increasing the possibility of undue derrick swaying and/or collapse. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the rig manufacturer load guying chart posted on the rig. Abatement Note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Union Drilling Company, Piceance Creek, Rig 47, Rifle, CO: On or about 6/9/2005, the employer did not ensure that an adequate "chain-around" system bypassed come-a-long devices used to tension four wind guys on the derrick. Whereas the come-a-longs are commonly used to tension windguys, they are not intended to transmit the load from the wind guy wire ropes to the anchors. Absence of an adequate "chain-around" bypass mechanism exposed employees to undue derrick swaying in the event of windguy failure and consequent "struck-by" hazards. Among other methods, one feasible and acceptable method to correct the hazard is to observe the come-a-long manufacturer's specifications that prescribe "chaining-around" the come-a-long tensioning system in the wind-guy anchor assembly.
