employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed arc flash and arc blast:
(a)Andersen Sales and Salvage Inc., at 1490 E. 8th St., Greeley, CO 80631:
On August 25, 2005 an employee was exposed to electrical hazards when
opening an electrical panel without performing a Flash Hazard Analysis.
The
employee opened the two operational disconnects with the external
disconnect
leaving the middle disconnect closed.  This procedure conducted without
performing a Flash Hazard Analysis exposed the employee to arc flash and
arc
blast when he approached the bus that was energized to 4160 Volts.
Abatement Note: National Fire Protection Association 70E paragraph 130.3
Flash Hazard
Analysis describes the flash hazard analysis process and equipment to
protect personnel from
the possibility of being injured by arc flash.  The analysis shall
determine the flash protection
boundry and the personal protective equipment that employees within the
flash protection
boundry shall use.

for this item (see
enclosed "Sample Abatement-Certification Letter").-Certification
