employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in

logs.
Each
plank was approximately one foot in width. Five individual noncontiguous
planks
were used to set header logs. Heights above ground level for each of the
five
planks ranged from approximately twelve to fifteen feet above the lower
level.
While workers were engaged in installing the header logs, the workers were
not
protected from the fall hazard. Guardrails or other fall protection
measures were
not provided for the employees. This condition exposed two employees to
approximate twelve foot through fifteen foot fall hazards.
