rig #5, the hydraulic Pull Master winch was used to elevate personnel
above the
drilling platform. This winch is not designed for operations involving
lifting or
moving personnel as indicated by the manufacturer.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is to read and understand the manufacturers instructions prior
to
operation thereby utilizing hoists designed for lifting and moving of
personnel.

8/30/2006, the employer's rough terrain forklift was operating with a
broken
safety device. The fork "stop" mechanism to prevent the fork from
inadvertently
coming off the mast carriage was broken off.  Condition exposed employees
to
struck by hazards.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is to ensure that a trained individual inspects the forks on
the
machine
as found in the American National Standards Institute (ANSI) B56.2005
edition
titled Safety Standards for Rough Terrain Forklift Trucks.
