Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fire and explosion hazards:
(a) Location - Cutter area, Dye House:
On or about 11/3/05, employees were exposed to fire and explosion hazards
while
working in a Class II, Division II hazardous location.  The employer had
not evaluated
cut flock material for combustible and explosive properties.
Among other methods, one feasible and acceptable means to correct this
hazard would
be: Conduct a hazard assessment inclusive of all uses of similar material
in
(vacuuming) of dust accumulations - with approved equipment.  Employ other
means to
minimize the risk of ignition from all possible sources. Include in the
assessment the
methods of fire detection and suppression extinguishing systems, and
venting systems,
among others. The employer should verify facility compliance with all
aspects of 29 CFR
1910.307 "Hazardous (classified) locations"; Ensure current fire
extinguisher training and
emergency preparedness.
Suitable electrical equipment designed for Class II locations should be
used where needed
based on the material's explosive properties as determined by the NMAB
353-3-80
testing;
Strict control of fugitive/stray fibers by enclosure of process equipment
should be
considered if determined not to increase the hazard potential; All
appropriate equipment
and components in the application/process area should be installed in
accordance
with
OSHA and NEC requirements.

(incompatible chemicals) together.

be to separate the chemicals and store them in accordance with the
manufacturers

utilizing a
4000-pound capacity Wright-Speedway hoist which was mounted to a 2000-pound
capacity steel monorail.
Among other methods, one feasible and acceptable means to abate the hazard
is to install
a properly-rated hoisting mechanism in accordance with ASME/ANSI
B30.16.2003.

frequent basis.

hazard is to
train employees to inspect the cranes and hoisting devices at the
beginning of a
workshift. Among the items to inspect include but are not limited to:  all
function
operating mechanisms, deterioration or leakage in lines, drums, valves,
drain pumps,
condition of wire rope, brakes, upper limit switch, etc.ch,
