Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a. On or about December 12, 2005, at the 8216 S. Racine Ave., Chicago, IL jobsite, the following conditions exited which exposed employees to struck by hazards in the event of concrete masonry units wall(s) structural failure: 1. 4th Floor- Grout was not placed in all required cells of concrete masonry unit walls which supported structural steel lintels upon which hollow core prefabricated concrete slabs were set. 2. 4th Floor- Reinforcement bars were not installed in all required cells of concrete masonry unit walls upon which prefabricated concrete slabs were set. Feasible and acceptable abatement methods include, but are not limited to the following: Adherence to the Matrix Engineering Corporation's Sheet S-500 structural design drawings developed for the project.d
