were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the danger created by the unguarded vision panels in the
hoistway doors while
riding in or waiting for the moving elevator:
(a)Try Street Terminal Site, Freight Elevator #3:  Unguarded vision panels
existed on the
hoistway doors on Floors 2 through 9 while the elevator operated, thereby
exposing employees
at the hoistway doors and inside the elevator to the danger posed by
putting body parts through
the unguarded openings, as observed 4/13/06.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to cover the openings created by the missing vision panels in the hoistway
doors in accordance
with the American National Standard, American Society of Mechanical
Engineers, "Safety Code
for Elevators and Escalators" ASME A17.1-1996, Section 110.7a.
