North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish to each of his employees conditions of employment
and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to
employees:
a)Facility - - The employer did not ensure that the three (3)
bottom-running cranes are inspected
frequently (daily to monthly) as required.
REFERENCE:ANSI/ASME B30.11-1973 - Monorail Systems and Underhung Cranes,
Chapter
11.2 - Inspection, Testing, and Maintenance:
Section 11.2.1.1 - Inspection Classification:
b)Inspection procedures for cranes and monorail track systems shall be
divided into two general classifications:
1)Frequent Inspection - daily to monthly intervals.
Section 11.2.1.2 - Frequent Inspection:
Items such as the following shall be inspected at intervals as specifically
indicated:
a)All functional operating mechanisms for maladjustment - daily.
b)All functional operating mechanisms for excessive wear - daily.
c)All safety devices for malfunctions - daily.
d)All hoist and travel limit switches [if present] - daily.
e)Deterioration or leakage in lines, tanks, valves, drain pumps, or other
parts of air or hydraulic systems - daily.
f)Frequent inspection of hooks as specified in ANSI/ASME B30.10.
g)Frequent inspection of wire rope or chains as per the manufacturer's
recommendations.
Section 11.2.1.5 - Inspection Records:
Written, dated, and signed inspection reports and records should be made on
critical items such as hooks, brakes, ropes, chains, trolley swivels, and
safety
devices.  Records should be readily available to appointed personnel.
NOTE:Among other methods, one feasible and acceptable method to correct
this hazard is to
establish a process for the frequent inspection of the three (3)
bottom-running cranes to
cover the required items by qualified individuals including procedures for
the
documentation of these inspections and the retention of these records.n

Section 11.2.1.1 - Inspection Classification:
b)Inspection procedures for cranes and monorail track systems shall be
divided into two general classifications:
2)Periodic Inspection - one to twelve month intervals.
Section 11.2.1.3 - Periodic Inspections:
Complete inspection of the crane or monorail system shall be performed at
intervals as generally defined in paragraph 11-2.1.1-b.2 depending upon it
activity, severity of service, and environment.  These inspections shall
include
the requirements of paragraph 11-2.1.2 and, in addition, items such as
listed
shall be carefully examined and a determination made as to whether they
constitute a safety hazard:
a)Loose bolts and rivets.
b)Deformed, cracked, or corroded structural members.
c)Cracked or worn sheaves.
d)Worn, cracked or distorted parts.
e)Wear on brake system parts, linings, pawls, and ratchets.
f)Wear of chain sprockets and stretch of chain.
g)Electrical apparatus for signs of pitting, or deterioration.
h)Worn drive tires.
i)Periodic inspection procedure for overhead cranes per ANSI/ASME
B30.16.
j)Periodic inspection procedure for hooks as per ANSI/ASME B30.10.
k)Wear on the lower load carrying flange of all track sections.
Section 11.2.1.5 - Inspection Records:
Written, dated, and signed inspection reports and records should be made on
critical items such as hooks, brakes, ropes, chains, trolley swivels, and
safety
devices.  Records should be readily available to appointed personnel.
