SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards
which may cause death or serious physical harm to his employees and comply
with this
regulation and other occupational safety and health rules and regulations
promulgated under
Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as
amended, as
follows:
A.  On or about June 21, 2005, employer knew or should have known that
employees
installing 24 metal plate connected wood trusses, in excess of 12 feet
above floor level at
church building, were exposed to the hazard of being struck by tresses
toppling and falling to
the floor due to only spacer blocks and sporadic lateral and cross bracing
used as temporary
bracing for the trusses.  A feasible and useful method to correct this
hazard, among other
methods, is to obtain and follow the instructions and recommendations in
the Commentary
and Recommendations for Handling, Installing, and Bracing Metal Plate
Connected Wood
Trusses HIB-91 or equivalent.
B.  On or about June 21, 2005, employer knew or should have known that
employees
installing 24 metal plate connected wood trusses, in excess of 12 feet
above floor and ground
level at church building, were exposed to the hazard of being struck by
truss components
during possible failure of a truss to maintain its structural integrity
due to each truss rigged
and hoisted without utilization of a spreader bar and with the hoisting
slings attached to the
top cord of each truss at the wrong angle.  A feasible and useful method
to correct this
hazard, among other methods, is to obtain and follow the instructions and
recommendations
in the Commentary and Recommendations for Handling, Installing, and
Bracing Metal Plate
Connected Wood Trusses HIB-91 or equivalent.
