A.  Employer knew or should have known that forklift operators using
equipment such as,
but not limited to, Taylor and Hyster forklifts were exposed to the hazard
of being crushed
by overhead cage of forklift, in the event of a tip over, while not
wearing seat belt on
forklift.  A feasible and useful means of abatement, among others, is to
require and enforce
the use of the provided seat belt.
