Komatsu forklift,
Serial Number 40493, were exposed to the hazard of being thrown from the
forklift and
crushed due to not wearing a seat belt.  A feasible and useful method to
correct the hazard,
among others, is to require the use of a seat belt on the forklift in the
yard of the
establishment.
