SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards
which may cause death or serious physical harm to his employees and comply
with this
regulation and other occupational safety and health rules and regulations
promulgated under
Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as
amended, as
follows:
A.  Employer knew or should have known that three employees in an elevated
work basket,
in excess of 10 feet high while the operator was not at the controls of a
Lull G44B-42 model
forklift, Serial Number 77212N21-214, at front of building, constituted a
hazard of the
employees being unable to come down or of the basket falling if the
controls failed.  A
useful and feasible method, among others, to correct this hazard, is to
have the operator
remain at the controls while employees are in the elevated basket.
