Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees a place of employment which was free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of a fall hazard when two employees
were allowed to
work from the top of the same extension ladder.
a)  Among other methods, one feasible and acceptable method to correct
this hazard is
to ensure that ladders selected for use are used as designed in accordance
with Section
8.2.1 of the American National Standards Institute (ANSI) A14.2-2000,
Safety
Requirements for Portable Metal Ladders where it states "Portable Ladders
are designed
for one-man use to meet the requirements of the man, the task, and the
environment.
