Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of: a) On March 21, 2006, the employer did not follow the manufacturer's recommenda- tions when using the Mazak Slant Turn 450 CNC Lathe #104. Pipe being threaded was not properly supported to prevent the pipe from oscillating or "whipping". Pipe (pup joints) vary in lengths from 4 to 20 ft., with approximately 5 ft. being secured within the machine by a set of two chucks. Pup joints with unsupported lengths exceeding 5 ft. have the potential for "whipping" and therefore expose nearby employees to being struck by the unsupported length. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to: Assure that the length of pipes extending past the rear of the machine are supported and/or restrained to prevent the pipe from "whipping".
