were exposed to the hazard of being crushed by the CAT 3500 powered
industrial forklift truck
in the event of a tip over accident.
Shipment department; A seat belt was not provided thus not being used by
operators of
the CAT 3500 powered industrial truck that was being used for a variety of
tasks. The
employees were  exposed to the hazards of being crushed by potential
roll-over action
of the fork trucks.
Among other methods, one feasible and acceptable method to correct this
hazard
