Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Fire/Explosion Hazards a) Worksite located at 306 Columbia Turnpike, Florham Park, NJ Employees were operating the VIM furnace when a steam explosion occurred due to molten metal leaking from the crucible and contacting the cooling lines. The crucible did not have a molten metal leak detector that would sound an audible alarm. Employees did not receive regular equipment/process specific training. The VIM furnace was not equipped with an audible ground fault alarm. Violation observed on or about 7/18/05 FEASIBLE AND ACCEPTABLE METHODS OF ABATEMENT, AMONG OTHERS, TO CORRECT THIS HAZARD ARE: Follow recommendations in accordance to the 1985 NFPA 86D, Industrial Furnaces Using Vacuum as an Atmosphere: a) Section 9-5.7, Ground fault detection system that sounds an alarm whenever molten metal gets close to the water cooling lines near the induction coils. b) Section 9-5.6, Molten metal leak detector that sounds an alarm when molten metal is sensed in the refractory indicating a leaking crucible. c) Section 1-9(c), Provide regular retraining of regular vacuum induction furnace operators in areas such as, but not limited to, those listed in Section 1-9(f).
