The Gemini and the Western Automotive Lifts in the maintenance bays had
swing  arm
restraints that were damaged or not functioning. The south lift arm(s) on
the Gemini Lift
were not level causing the suspended vehicle to tilt. The anchor bolts on
the south post
of the Gemini lift were not installed according to the manufacturer's
recommendations.
Periodic inspection and preventive maintenance procedures were not
established; and a
thorough record of each repair maintenance procedure performed was not
prepared and
maintained.
