Minn. Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: The Gemini and the Western Automotive Lifts in the maintenance bays had swing arm restraints that were damaged or not functioning. The south lift arm(s) on the Gemini Lift were not level causing the suspended vehicle to tilt. The anchor bolts on the south post of the Gemini lift were not installed according to the manufacturer's recommendations. Periodic inspection and preventive maintenance procedures were not established; and a thorough record of each repair maintenance procedure performed was not prepared and maintained. Abatement Guidelines: Refer to American National Standards Institute Standard ANSI/ALI ALOIM-2000 Standard for Automotive Lifts - Safety Requirements for Operation, Inspection and Maintenance; and the manufacturer's recommendations. The ANSI standard contains additional detail, including checklists and forms for lift inspections and maintenance that may be helpful.
