were causing or likely to cause death or serious physical harm to
employees in that the
employees were exposed to being crushed by shifting Hydraulic Drilling Rig
while changing drill
bits while resting unsecured on track cart.
a)On site.
Employees working changing drill bits from ICI Model 308 Hydraulic
Piling/Drilling Rig
while it was unsecured on a track cart, were not protected from the hazard
of the drill
rig falling over, on or about 6/22/05.
ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS, TO
CORRECT THIS HAZARD IS: to follow the manufacturer`s manual, "Safety
Notes/Transportation/Movements" When traveling to a work site the mast
must be in the
horizontal position. The maximum gradient that can be tackled by the rig
is
10 degrees in
longitudinal and transversal directions.  The machine can normally only
operate on level ground.
If absolutely necessary, the machine can be used on a slight gradient
provided it remains  within
specified stability limits of 3 degrees max.
NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION
AND DOCUMENTATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19NCE
