Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught in, fall, and crushing hazards: a) On or about 8/12/05, in the waste paper recycling area, employees were walking and/or riding on a moving conveyor belt to remove jams from the feed hopper for a Balemaster Baler. The conveyor was not equiped with an emergency stopping device in the immediate vicinity of the employee at the top of the conveyor, allowing the employee to be carried into the baler if the employee were to fall. Among others, a feasible method to eliminate or reduce this hazard; a) Comply with ANSI B20.1-1996 section 5.12(d) safe operating procedures prohibiting employees from walking / working on moving belts b) Comply with ANSI B20.1-1996 section 5.11.2(c)(1) emergency stop devices shall be easily identifiale and in the immediate vicinity c) Comply with ANSI B20.1-1996 section 5.12 Operation, only a trained person shall be permitted to operate a conveyor. ABATEMENT DOCUMENTATION REQUIRED
