that were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck by hazards from loads that were carried by the lift
master boom model
LMEBT4 or LMEBT6 which was not securely attached to the forklift carriage.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to:
Follow the manufacturers manual which states to secure the boom to the
forklift before operating the
device, and the warning labels which state,  WARNING: Securely attach the
safety locking device to
the forklift.
