Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards created by operating a printing press without
proper training and
standardized operating procedures:
Press #8 - On 9/2/05, an employee was fatally injured while helping
perform a manual
roll change on a printing press.  The employee got caught between a roll
of paper and
the framework of the roll stand as the turret was rotating a new roll of
paper into
position. There are no written operating procedures for any of the tasks
related to
operating the press.  Training is on the job training performed by the
person with the
"next level" of experience on the press ( i.e. the roll tender trains the
stacker and the
head pressman trains the 2nd pressman). There are four shifts and six
presses.
As most
people have their own way of doing things and the fact that the presses
are not identical,
with each having slightly different safety and operating features, this
presents a problem
in insuring adequate training for the press operating personnel.
Among other methods, one feasible and acceptable method to correct the
hazard is to
develop standard operating procedures and train employees based upon those
procedures.
