KRS 338.031(1)(a): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a.A roof collapse from contacting a buildings column roof support pole multiple times with a forklift when conducting mold changes at the Mitsubishi horizontal injection molding machine #16 in Building #2's Production Area. One feasible and acceptable abatement method, among others, to correct this hazard, is to move the Mitsubishi horizontal injection molding machine #16 so that the forklift is no longer operating in a tight area where the column support pole is struck, when conducting mold changes. in the alternative: SEE CITATION 1 ITEM 1B

KRS 338.031(1)(a): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a. A roof collapse after attempting to replace a damaged structural roof support column that had been repeatedly struck by a forklift during mold changes at the Mitsubishi horizontal injection molding machine #16 in Building #2's Production Area, without a professional engineer and/or architect review and/or approval. One feasible and acceptable abatement method, among others, to correct this hazard, would be to have a professional engineer and/or architect review the damage, and approve any repair as stipulated in the 2002 Kentucky Building Code.
