KRS 338.031(1)(a): Each employer did not furnish to each of his employees employment and a place of employment which was free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees: a.United Parcel Service had replacement hydraulic hose assemblies manufactured for the four Rediffusion flight simulators, such as but not limited to S03 757 Pratt FAA ID # 318 at the Flight Simulator Building, that were not pressure tested and did not meet the simulator manufacturers testing requirements. The hazard is that hoses put into service without prior pressure testing could burst or fail, exposing employees to high pressure hydraulic fluid and flying hose and coupling fragments. One feasible and acceptable abatement method, among others, to correct this hazard is testing the hydraulic hose assemblies with the application of hydraulic fluid pressure prior to service.
