Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees, employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: (a) On or about August 30, 2006 - An employee was exposed to a fall hazard exceeding 80 feet while using a rope descent system with a lifeline that had worn fibers due to excessive abrasion. Among other methods of abatement, a feasible and acceptable method to correct this hazard is to follow the procedures found in the American National Standard Institute (ANSI) Standard IWCA I-14.1-2001, paragraph 14.5.7., which requires that ropes be removed from service when excessive abrasion has worn fibers. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees, employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: (a) On or about August 30, 2006 - A portable metal ladder, used to access the roof of a canopy, was placed with the top of the ladder approximately 1 foot below the roof line, exposing the employee to a fall hazard of up to 12 feet to the next lower level. Among other methods of abatement, one feasible and acceptable method to correct this hazard is to provide longer ladders and required employees to use ladders as prescribed in the American National Standards Institute (ANSI) Standard A14.2 1990. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
