Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed being struck due to parting of the load line on a crane and the falling of the load hook and load line weight (headache ball): a) On or about December 4, 2006, the anti-two-blocking device on the load line of the truck mounted USTC 1500 hydraulic crane was damaged and had been completely disconnected preventing the actuation of the safety system which would prevent a two- blocking accident. One suitable and acceptable means of abatement of this hazard is to comply with Section 5-1.9.9(a) of the ANSI B30.5, 1994 Mobile and Locomotive Cranes Standard and repair or replace the manufacturer's installed and required anti-two-blocking device. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIREDATION
