Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a tire explosion when the wrong size tire machine with a inoperable pressure gauge was being used. In an effort to seat the tire on the rim, the tire was inflated until the tire exploded with the employees face in close proximity. (a) Location - service area, tire mounting machine: On or about 07/09/2005, employees were exposed to explosion hazards while mounting tires onto rims/wheels with a machine that was not designed to accommodate the size wheel that was being worked on, which also had a inoperable air pressure gauge. Among other feasible means of abatement, one acceptable method to correct the hazard is to: Operate and maintain the Monty tire mounting machine within the scope of the manufacturer's instructions and instruct each employee in the safe operation of the equipment.
