recognized hazards that

confined space:
a) Location - St. John's Prep School, Spring & Summer Streets, Danvers:
On or about August 4, 2005, an employee was performing hot work
(soldering) inside
a manhole without an attendant as part of a confined space entry program.
Among other methods, feasible means of abatement to correct this hazard is:
1) Develop, implement, and complete a confined space permit entry system
meeting the
requirements of OSHA 1910.146 and/or ANSI Z117.1-1989.
2) The use of an effective retrieval/rescue method, i.e., mechanical means
that would
include a fall arrest system, where appropriate, for employees for
entry/rescue/exit.
3) Provision of an effective means of communication by an attendant to
necessary rescue
services.

there is a
potential for oxygen deficiency or entry of toxic or hazardous air
contaminants.
