Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to fall and struck by hazards: Dollar General Store
Jobsite, Staunton
IL, on or about August 14, 2008, the bracing of long span supporting
trusses was inadequate
resulting in the collapse of the truss system during installation.  The
wooden trusses were not
installed following the recommendations of the Wooden Truss Council of
America's
Summary Sheets BCSI B2 "Truss Installation & Temporary Restraint/Bracing"
in the
following area: (1) the lack of temporary bracing of the trusses was
contrary to the
recommendations of the BCSI, and (2) the trusses were hoisted on to the
roof not following
the recommendations of the Wooden Truss Council of America's Summary
Sheets BCSI B1
"Guide for Handling, Installing, Restraining, and Bracing of Trusses" in
that
trusses were
set using a 26 foot long stiff-back, which was not at least 2/3 to 3/4 the
length of the trusses
being set.
Among other methods, one feasible and acceptable method of abatement would
be (1) to
comply with the BCSI B2 Summary Sheet "Truss Installation & Temporary
Restraint/Bracing" and follow the temporary bracing design recommendations
which require
consultation with a qualified professional engineer for all temporary
bracing systems for truss
spans over 60 feet and (2) to comply with B-1 requiring the use of a
stiff-arm/spreader beam
which follows the recommendations of the B-1 Summary Sheet "Guide for
Handling,
Installing, Restraining, and Bracing of Trusses" by using a stiff-back or
spreader beam which
was 2/3 to 3/4 the length of the trusses.
ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS ITEM IN ACCORDANCE
WITH THE REQUIREMENTS OF 29 CFR 1903.19(d).S
