that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to struck-by hazards:
a) Repair Shop Small Parts Room - The employer did not ensure that when a
repair was
made to an Emglo air receiver (pressure vessel) by welding, the repair was
made as
prescribed by the American National Standards Institute/National Board
Inspection Code,
ANSI/NB-23 (1992), Chapter III, which requires in part that no repair to a
boiler or pressure
vessel shall be initiated without the authorization of the Inspector who
shall satisfy himself
that the welding procedures and welders are qualified and that the repair
methods are
acceptable.
Among other methods, feasible and acceptable means of abatement to correct
this
hazard
include, but are not limited to the following:
Ensure that welded repairs conform to The American National Standards
Institute/National
Board Inspection Code "A Manual for Boiler and Pressure Vessel
Inspectors", ANSI/NB-23
(1992), Chapter III.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this
violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET),
and in addition, documentation demonstrating that abatement is complete
must be
included with your certification.  This documentation may include, but is
not limited to,
evidence of the purchase or repair of the equipment, photographic or video
evidence of
abatement, or other written records.
