recognized hazards that were causing or likely to cause death or serious
physical harm to employees in that

a)shop - where employees were allowed to use  a  2 ton electric chain
hoist to lift miscellaneous
metal parts for the plasma cutter table, the safety latch was missing on
the hook and the rail was
not load rated. The hoist had not been inspected in the past 12 months.
