North Carolina General Statute 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish to
each of his employees  conditions of employment and a place of
employment free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees, in that
employees were exposed to a fall due to an improper job made
ladder: (ref para 4.5.1, 4.5.2, and 4.5.5, ANSI A14.4-1979):
a)2219 Clayette Ct.-employees were accessing the upper
level of a home under construction using a job made
ladder which did not meet the provisions of paras 4.5.1,
4.5.2, and 4.5.5 ANSI A14.4-1979, in that the cleats were
not properly attached, filler blocks were not installed
on the ladder, and the spacing of the cleats exceeded 12"
+- 1/2".
