employer did not furnish each of his employees conditions of employment
and a place of employment free from
recognized hazards that were causing or likely to cause death or serious
physical harm to employees in that
employees were exposed to:
a)facility - where the safety latch was missing from the hook of the chain
hoist used to suspend
the  radiator (which weighed approximately 250 pounds) of a school bus.
One feasible abatement method, among many, is to place a safety latch on
the hook (in
accordance with ASME B30.16-1993) in order to prevent the suspended
equipment from falling
from the hook.
