North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)facility - where the safety latch was missing from the hook of the chain hoist used to suspend the radiator (which weighed approximately 250 pounds) of a school bus. One feasible abatement method, among many, is to place a safety latch on the hook (in accordance with ASME B30.16-1993) in order to prevent the suspended equipment from falling from the hook. (Reference: Section16-1.2.9 Hooks of the American Society of Mechanical Engineers (ASME) B30.16-1993 Overhead Hoists (Underhung).

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)facility - where the employer did not have a certification record of inspections. One feasible abatement method, among many, is to implement an operator hoist inspection list before use and frequently throughout the year (in accordance with ANSI B30.11-1980) in order to prevent the wear, tear, and distortion or stretched links and damaged hooks. (Reference: Section 11-2.1.4(a) of American National Standards Institute (ANSI) B30.11-1980::Monorails and Underhung Cranes)
