Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to catastrophic failure of a wellhead assembly resulting in
an explosion and fire:
a) Forest County, Howe Township, Madison #35 Well - On or about July 29,
2006 the
stripper rubber component of the Lowery L-HFCS-W wellhead assembly failed
when
circulation was reversed. This component is not designed to retain the
full working
pressure of the entire wellhead assembly, unless the package parts have
been installed
and the complete unit assembled.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to: 1. Follow proper installation procedures according to the
Lowery Well
Heads, Inc. model L-HFCS-W model wellhead. 2. Ensure in normal operations,
pumps,
piping, hoses, valves, and other fittings shall not be operated at
pressures greater than
their rated working pressure and shall be maintained in good operating
condition, as
specified by Section 9.13.4 of the American Petroleum Institute (API)
Standard RP 54-
1999, "Recommended Practices for Occupational Safety and Health for Oil
and Gas Well
Drilling and Servicing Operations."
