Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to an uncontrolled release of anhydrous ammonia:
a) Machinery Room - On or about January 29, 2007 at Pellegrino Food
Products Co.,
Inc. employees working in and adjacent to the machinery room were exposed
to a
potential release of anhydrous ammonia from the refrigeration system used
at the facility.
Feasible methods of abatement include, but are not limited to the
following; 1. Label and
identify system piping, valves, and switches. 2. Install entry doors that
are tight-fitting,
self-closing, and fire-rated equipped with panic-type hardware. 3. Conduct
an annual
inspection of all anhydrous ammonia system components including vessels
and heat
exchanger. 4. Close any wall penetrations which would allow refrigerant
leakage
to other
parts of the building. 5. Install self-closing or manual quick-closing
emergency stop
valves on the oil pot drain. 6. Determine and identify the five (5) year
service
replacement dates on all pressure relief valves.
