1.3.2 SUPPORT, IT STATES "THE SUPPORTING STRUCTURE, INCLUDING
TROLLEYS, MONORAIL, OR CRANE, SHALL BE DESIGNED TO WITHSTAND THE
LOADS AND FORCES IMPOSED BY THE HOIST FOR THE RATED LOAD."

company were using a hoist that did not have its pendant controls
marked as to which direction control buttons would make hook travel.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD OF
ABATEMENT WOULD BE TO FOLLOW THE RECOMMENDATIONS OUTLINED IN
ASME B30.16-2003, "OVERHEAD HOISTS (UNDERHUNG)". UNDER SECTION 16-
1.1.2 CONTROLS, IT STATES "EACH CONTROL ACTUATOR OF AN ELECTRIC OR
AIR POWERED HOIST SHALL BE MARKED TO INDICATE THE DIRECTION OF
RESULTANT MOTION."

a)New Building - On or about April 24, 2007, employees of the company
were using a 3 ton overhead hoist to move forms to and from a
Concrete Block Machine. When lowering a load with the hoist, load
would continue in the downward motion after downward button on
control was released. Upward button on control had to be pressed to
stop downward motion or load would continue all the way to the floor.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD OF
ABATEMENT WOULD BE TO FOLLOW THE RECOMMENDATIONS OUTLINED IN
ASME B30.16-2003, "OVERHEAD HOISTS (UNDERHUNG)". UNDER SECTIONS 16-
2.1.2, FREQUENT INSPECTION, PARAGRAPH (C), STATES THAT "THE
FOLLOWING ITEMS SHALL BE INSPECTED: (3) HOIST BRAKING SYSTEM FOR
PROPER OPERATION."
