furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a hazard of being struck by falling drill pipe sections:
a) Clearfield County, Chest Township, Frailey #50 Well - On or about April
18, 2007
the employer did not ensure that the U-bolt wire rope clamps for the
hoisting line (cat
line) were installed in accordance with the manufacturers instructions (U
section contacts
dead end).
ABATEMENT NOTE:
Among other methods, feasible and acceptable method of abatement:
1. Follow the American Petroleum Institute (API) RP 9B, August 2002,
Recommendation
Practice on Application, Care, and Use of Wire Rope for Oilfield Service.
2. Follow the U-bolt manufacturers (Crosby) installation instructions.
