Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the potential for blunt force trauma from an explosion or sudden pressure release due to a failed weld: a)Mercer Forge Company, forging press #980362 - On or about, 2/25/08, an employee had performed welding on a compressed air tank. ABATEMENT NOTE: Among other methods, feasible and acceptable means of abatement to correct this hazard include, but are not limited to the following: 1) Obtain manufacturer's approval to weld on the vessel, 2) Ensure the welding is only performed by someone who is certified in pressure vessel welding, 3) Have pressure vessel inspected by a certified pressure vessel inspector.
