Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to operating a drilling rig with improper rig stability configuration: a) Jones Unit 3, Woodcock Twp, Crawford Cnty, PA - On or about June 12, 2008 the right side forward crown guy wire was anchored to a twelve inch (12") maple tree. b) Jones Unit 3, Woodcock Twp, Crawford Cnty, PA - On or about June 12, 2008 the left side forward crown guy wire was anchored to a nineteen inch (19") diameter maple tree. c) Jones Unit 3, Woodcock Twp, Crawford Cnty, PA - On or about June 12, 2008 the right side rear crown guy wire was anchored to a sixteen inch (16") diameter maple tree. Also, the employer did not ensure a "chain-around" system bypassed the come-a-long that was used to tension the guy wire. d) Jones Unit 3, Woodcock Twp, Crawford Cnty, PA - On or about June 12, 2008 the left side rear crown guy wire was anchored to an eleven inch (11") maple tree. e)Jones Unit 3, Woodcock Twp, Crawford Cnty, PA - On or about June 12, 2008 the right side racking board guy wire was anchored to the pipe tub. f) Jones Unit 3, Woodcock Twp, Crawford Cnty, PA - On or about June 12, 2008 the left side racking board guy wire was anchored to the pipe tub. ABATEMENT NOTE: Among others, one feasible and useful abatement method to correct this hazard is to: (1) Follow the requirements of Wilson Manufacturing Co. Guyline Arrangement specification (which replicates the posted placard mounted on the rig provided by the manufacturer) and American Petroleum Institute (API ) 4E Drilling and Well Servicing Structures (2) Utilize the come-a-long manufacturer's specifications that prescribe "chaining around" the come-a-long tension system in the guy wire/anchor assemblyhor

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to working from an elevated work platform without properly installed Saf-T-Slide emergency escape line: a) Jones Unit 3, Woodcock Twp, Crawford Cnty, PA - On or about June 12, 2008 the emergency escape line for the Saf-T-Slide device was improperly anchored. Manufacturer instructions prohibit the use of stakes, trees stumps or any object such as a drum of cement. The ground anchor must be able to withstand a 3,000 lb. load. ABATEMENT REFERENCE: Reference Melco Saf-T-Slide Operators Manual, Ground Anchor

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to high pressure hoses and flow lines not secured to prevent whipping action in the event of failure: a) Jones Unit 3, Woodcock Twp, Crawford Cnty, PA - On or about June 12, 2008 two (2) three inch (3") high pressure hoses were not hobbled with safety chains to prevent whipping action in the event of hose failure. b) Jones Unit 3, Woodcock Twp, Crawford Cnty, PA - On or about June 12, 2008 the main flow line was not secured with tie downs (chain and binder) on each side to prevent movement. The flow line was not mounted or supported properly to prevent collapse. ABATEMENT NOTE: Reference American Petroleum Institute (API) RP 54 Section 9.13 Pressure Equipment
