Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to physical harm from crushing injuries from derrick failure or collapse: a) Trojack Unit 1, Union Twp, Erie Cnty, PA - On or about June 23, 2008 the company did not stabilize the 2007 Dickirson SR-60 Double Drum Rig #4550 or company number SE #2 according to manufacturer's guyline/anchor pattern. Eight (8) external guylines and four (4) ground anchors were not set to prevent collapse or mast failure. ABATEMENT NOTE: Among others, one feasible and useful abatement method to correct this hazard is to: (1) Follow the requirements of Dickirson Corporation specification sheet (which replicates the posted placard mounted on the rig provided by the manufacturer) which requires eight (8) guy wires from the derrick to be connected to four (4) ground anchors; (2) Reference American Petroleum Institute (API) Recommended Practice RP54 Paragraph 6.3.2 which states that "Well operations involving the rig shall not be commenced until the rig is rigged up in a safe manner"; (3) Reference OSHA Instruction PUB 8-1.8 Guidelines on the Stability of Well Servicing Derricks dated July 15, 1991; (4) Reference Association of Energy Service Companies (AESC) Recommended Safe Procedures and Guidelines for Oil and Gas Well Servicing page 67 "Every rig should be equipped with the number and size of guy lines as recommended by the manufacturer. Every guy line recommended by the manufacturer should be utilized according to the manufacturer's specification".
