Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck by hazards:
On or about October 19,2005, employees engaged in depressurizing a
hydrostatic
test manifold, depressurized too quickly resulting in the valve separating
from the
test manifold and becoming airborne.
Among other feasible abatement alternatives:
a.The employer could include depressurization procedures in the written
"hydrostatic test plan."
b.The employer could have written standard operating procedures
concerning the selection and placement of valves and fittings in and on the
hydrostatic test manifold.
c.The employer could have a written "job safety analysis" for the
depressurization portion of the work.on
