Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to multiple injuries from falling hazard when employees are
elevated on platforms
when used with a powered industrial truck and the platform is not secured
to the lifting carriage
and/or forks.  The gate was not completely provided with a midrail:
Reference:  Elevated Work Platform mounted on power industrial truck did
not comply with the
following ANSI B56.1-2000 Sections 7.36.1 and 7.36.3 requirements:
Facility:
(a) The personnel platform was not secured to the powered industrial
trucks lifting carriage
and/or forks and did not have a midrail on the gate.
(b) The platform did not have capacity ratings.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to follow ANSI B56.1-2000.
a.  The work platforms shall have permanent, legible identification
providing information of
platform weight, platform rated load, name and address of the
manufacturer.
b.  The work platform shall be secured to the fork carriage to prevent
forward, lateral or
rotational movement. The platforms should not protrude beyond the edge of
the fork arms.
c.  The platform shall have a midrail on the gate and 4 inch high toe
board on all sides, to
include the gate.
