Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling: a)On or about 10/05/05, Performing Arts Center: The Genie S-65 aerial lift had two of its tires set up on a series of boards, not a firm surface, with a drop off immediately to the North and East. Among other feasible abatement methods that could be used: 1) Utilize firm secure cribbing. 2) Utilize a tire stop/ put-log in front of the tires to prevent inadvertent movement. 3) Follow all of the requirements of the ANSI A92.5 Standard, for Boom Supported Elevated Work Platforms, including, but not limited to sections 13.1.3 and 13.2.2.
