were exposed to THE HAZARD OF BEING STRUCK-BY:
a)  On or about 11/3/05 in the Receiving Area:  The overhead Load Lifter 1
1/2 ton
hoist had a damaged safety latch.
Among other methods, feasible and acceptable abatement methods to correct
to the hazard
are to:
1.  Install new safety latch on the lifting hooks in accordance with ANSI
B30.16-
1993, Section 16-1.2.9.
b)  On or about 11/3/05 in the Receiving Area:  The supporting rail for
the
overhead
Load Lifter 1 1/2 ton hoist had no load rating or capacity markings on it.
Among other methods, feasible and acceptable abatement methods to correct
to the hazard
are to:
1.  Prior to installation and use, ensure the supporting structure is
designed to
withstand the loads and forces imposed by the hoist for the rated load in
accordance
with ANSI B30.11-1980, Section 11-2.2.2.
2.  Mark the rated load on the rail in accordance with ANSI B30.11-1980,
Section
11-1.1.
Abatement documentation must be submitted for this item.
