Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a) The following crane structures were not clearly marked with the rated capacity for the supporting structure: 1. On or about 11/15/05 in the Coater Room: The #11 coater rewind monorail with a 2 ton hoist did not have the capacity clearly marked on the support structure. 2. On or about 11/15/05 in the Coater Room: The #11 coater unwind monorail with a 3 ton hoist did not have the capacity clearly marked on the support structure. 3. On or about 11/15/05 in the Coater Room: The #12 coater unwind monorail with a 2 ton hoist did not have the capacity clearly marked on the support structure. Among other methods, feasible and acceptable abatement methods to correct these hazards are to: 1.Prior to installation and use, ensure the supporting structure is designed to withstand the loads and forces imposed by the hoist for the rated load in accordance with ANSI B30.11-1980, Section 11-2.2.2. 2.Mark the rated load capacities on the supporting structures in accordance with ANSI B30.11-1980, Section 11-1.1. b)The following below-the-hook lifting devices had no rated load capacity markings and no inspections were conducted on the devices: 1.In the Coater Room: The below the hook lifting device used to load the paper reams was not rated and marked by a PE. 2.In the Frame Room: The below the hook lifting device used to load ream onto framer was not rated and marked by a PE. Among other methods, feasible and acceptable abatement methods to correct these hazards are to: 1.Have the rated load of the lifting device determined and marked on the main structure or equivalent in accordance with ASME B30.20-1993, Section 20- 1.2.1. 2.Conduct frequent and periodic inspections in accordance with ASME B30.20- 1993, Section 20-1.3 and make repairs accordingly.
