Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Impeller Floor, Roll-Over Area, on or about 11/29/05: Employees using a Ingersoll Rand one ton capacity air-powered wire rope hoist to lift poured molds weighing approximately 500lbs. Hoist was not being inspected using the frequent and periodic procedures as specified in American Society of Mechanical Engineers ASME B30.16-1993. ABATEMENT NOTE: One means of abatement would be to ensure that the Ingersoll Rand wire rope hoist be inspected per American Society of Mechanical Engineers, ASME B30.16-1993, Chapter 16-2, Sections 16-2.1.1, 16-2.1.2 and 16-2.1.3. b)Core Room, on or about 11/29/05: Employee using a 500lb.capacity air - powered chain hoist to lift cores weighing up to 300lbs. Hoist was not being inspected using the frequent and periodic procedures as specified in American Society of Mechanical Engineers ASME B30.16-1993. ABATEMENT NOTE: One means of abatement would be to ensure that the air-powered chain hoist be inspected per American Society of Mechanical Engineers, ASME B30.16-1993, Chapter 16-2, Sections 16-2.1.1, 16-2.1.2 and 16-2.1.3. c)Impeller Floor, Roll-Over Area, on or about 11/29/05: The supporting structure including monorail for the Ingersoll Rand one ton capacity air- powered wire rope hoist was not designed nor rated to with stand the loads and forces imposed by the hoist for the rated load. Hoist is used to lift molds weighing approximately 500lbs. ABATEMENT NOTE: One means of abatement would be to have a qualified person review the design of the supporting structure and establish rated load capacity as per American Society of Mechanical Engineers, ASME B30.16- 1993, Chapter 16-1, Section 16-1.3.2. d)Impeller Floor, Roll-Over Area, on or about 11/29/05: Ingersoll Rand one ton capacity air-powered wire rope hoist used by employees to lift molds, the upper limit device was inoperative in that employee could accidentally raise the load block into the hoist. ABATEMENT NOTE: One means of abatement would be to follow the procedure addressed in American Society of Mechanical Engineers, ASMEB30.16-1993, Chapter 16-2, Section 16-2.1.2(c)(2), hoist upper limit device is inspected at the beginning of each shift. e)Impeller Floor, Roll-Over Area, on or about 11/29/05: One below-the-hook lifting device used to support molds and attach said load to a Ingersoll Rand one ton capacity air-powered hoist was not marked as to its rated capacity. ABATEMENT NOTE: One means of abatement is to have a qualified person review the design of th below-the-hook device and have its established capacity marked on the device per American Society of Mechanical Engineers ASME B30.20-1993, Chapter 20-1, Section 20-1.2.1. f)Impeller Floor, Roll-Over Area, on or about 11/29/05: One below-the-hook lifting device used to support molds and attach said load to a Ingersoll Rand one ton capacity air-powered hoist was not being inspected using the frequent and periodic procedures as specified in American Society of Mechanical Engineers ASME B30.20-1993. ABATEMENT NOTE: One means of abatement would to ensure that the below-the-hook lifting device be inspected per American Society of Mechanical Engineers ASME B30.20-1993, Chapter 20-1, Section 20-1.3.1.
