Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to A FALL HAZARD: a)At the job site, on or about 12/22/2005: A JLG 600A, Boom-Supported Elevating Work Platform Aerial lift, number AR198279, was being used by two employees to attach plastic sheeting at a height of approximately 60 feet, from the ground, and the boom was being operated while parked on an unlevel and inclined surface approximately 10 inches from a drop off creating an unstable chassis allowing the wheels to lift off the ground. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: Ensure that all the manufacturer's safety requirements of operating the aerial lift on smooth firm level surfaces be adhered to in accordance with the manufacturer's Operation and Safety handbook,the Operator's Safety Manual and paragraphs 7 and 8, ANSI/SIA A92.5-1992. b)At the job site, on or about 12/22/2005: A JLG 600A, Boom-Supported Elevating Work Platform Aerial lift, number AR198279, was being used by two employees, to attach plastic sheeting at a height of approximately 60 feet from the ground,and they had not been trained to perform a workplace inspection and hazard recognition prior to the operation of the lift. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: Ensure employees are properly trained in accordance with the manufacturer's specifications and safety procedures to conduct a workplace inspection prior to the operation of an aerial lift in accordance with the manufacturer's Operation and Safety handbook,the Operator's Safety Manual and paragraphs 7 and 8, ANSI/SIA A92.5-1992.
